2. Scope of Services

Braintree Health Partners provides:

• BrainBoost: Personal injury–focused TBI diagnostics and neurological evaluations including QEEG, Oculogica EyeBox, and cognitive testing.

• RenewO₂: Cash-based wellness services such as Hyperbaric Oxygen Therapy (HBOT), peptide therapy, and hormone optimization.

These services are offered at our owned and affiliated clinical sites (“Access Points”) and may include physician oversight, third-party equipment, and independent contractor providers.

All information provided through Braintree websites, marketing materials, or communications is for informational and educational purposes only.

4. Patient Responsibilities

By engaging our services, you agree to:

5. Legal and Financial Responsibility

a. Personal Injury Cases

For diagnostic services performed under Letter of Protection (LOP) or Assignment of Benefits (AOB):

• You authorize Braintree to bill directly to your attorney or insurance settlement.

• You understand that ultimate responsibility for payment remains yours if settlement or recovery is insufficient.

• LOP or AOB documents must be executed before diagnostic services are rendered.

b. Wellness and Cash-Based Services

Payment for RenewO₂ services is due at the time of service unless otherwise stated.

All sales are final unless prohibited by law. No refunds are provided for partially used sessions or memberships.

6. Independent Practitioners and Partners

Braintree partners with independent chiropractors, physicians, and medical professionals (“Affiliates”) who may lease or host diagnostic equipment.

These affiliates are independent contractors, not employees of Braintree.

Braintree is not liable for acts, omissions, or professional decisions made by independent providers practicing within their own licensed scope.

7. Intellectual Property

All content, materials, graphics, videos, trademarks, and logos on Braintree’s platforms—including the names Braintree Health Partners, BrainBoost, and RenewO₂—are the property of Braintree or its licensors.

You may not copy, distribute, modify, or use any materials without prior written consent.

a. Use of Website

You agree to use our website and online tools only for lawful purposes.

You may not attempt to:

• Hack, disrupt, or gain unauthorized access to our systems

These Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict of law principles.

Any dispute shall be brought exclusively in the appropriate courts located in Pinellas County, Florida.

13. Indemnification

You agree to indemnify and hold harmless Braintree, its officers, employees, affiliates, and contractors from any claims, damages, or expenses (including attorney’s fees) arising out of:

• Your use of our services or website

• Violation of these Terms

• Misuse of personal injury documentation (LOP/AOB)
